ExtraSure
Travel Insurance Claims Procedures
In
the event of such claims, the policyholders/insured shall:
- Notify
UOI's claims Department of the occurence immediately upon
your return to Singapore.
- Submit
the air-ticket counterfoil, passenger receipt or tour booking
invoice and original ExtraSure Certificate of Insurance.
- Obtain
the claim form (PDF: 446 KB) from UOI, complete and return
the same to us within 10 days.
- For
luggage delay/damage claims, submit to UOI the Property
Irregularity Report/ Property Damage Report and Luggage
Tags.
- For
travel delay claims, submit to UOI the relevant Airline's
confirmation letter stating the reason and duration of delay.
- Submit
to UOI a copy of the police report if the loss occured whilst
not in the custody of the airlines and Transportation Authorities.
- Assist
and co-operate with UOI's appointed representatives at 6490
